jquery - Form submits, but doesnt send form data when zipcode modal submits -

if go provided link, can see form on sidebar "contact dealer". thought script check see if have provided zipcode exists in 1 of arrays.

if zipcode in either array, submits info appropriate php link file. feature works perfectly, , sends info email.

if zipcode not in 1 of arrays trigger modal window allows user pick dealer want send form info to. feature appears work, when receive info in email, sends empty lead setup. have provided script using submit within modal, modal popup.

if has thought of prepare appreciate it.
